Published under a Creative Commons Attribution 4.0 licence: use it commercially or otherwise, but credit the source and keep the negotiated-versus-cash distinction intact if you republish. Misrepresenting negotiated rates as cash prices is the specific error this dataset exists to avoid.
Underlying source files are public and listed on the sources page. Procedure code numbers are used as factual identifiers only; no code-set descriptor text is reproduced here.
