Observed dataObserved negotiated rates. Every figure here is a real negotiated amount taken from payer files published under the federal Transparency in Coverage rule. These are what a dental plan pays a contracted practice - not a cash price.

Supporting registries
- NPPES — the CMS National Plan & Provider Enumeration System, used to resolve each provider group to a practice location and dental specialty.
- Medicare Fee-For-Service Public Provider Enrollment — a CMS bulk extract used as a state-level fallback where NPPES has no record. It gives a state but no city, so practices resolved this way never appear in a city figure.
- HRSA Health Center Service Delivery and Look-Alike Sites — the federal list of funded health centres, used for the sliding-scale clinic listings on city and state pages. This dataset records locations, not services, so it cannot tell us which sites provide dentistry and we do not claim that it does.
Rate data and provider locations are the only inputs to the published figures. We use no licensed fee surveys and no commercial estimates — see methodology.
